PCO Drivers Medical Check

PCO Form Application £150 Only

PCO Medical Checks will be held at our main office in Central London. Please arrive 5 minutes prior to your appointment.

YOU MUST ENSURE YOU BRING THE FOLLOWING DOCUMENTS WITH YOU:

    • * Valid Driving License 
    • * TPH/204 Form

* FULL Medical Notes (Summary print outs will not be accepted) Your notes can be requested from your NSH GP.

If you are unable to provide these then this will be documented in the declaration section of the form.
 
Please note you will need to have the following: Medical conditions which could affect your application: Insulin treated diabetes, Epilepsy, Poor Vision, History of severe mental health problems (including drug or alcohol abuse), Heart problems, Neurological or neurosurgical disorders (strokes, blackouts, head injuries), Certain prescribed medication.
